Step 1 - Appoint us
Download and sign the Doctors’ Choice Appointment Letter.
Step 2 - Provide your information
Download, complete and sign the Doctors’ Choice Insurance Information Form which is relevant to you (Specialist, GP, PGY/Doctors in Training, Student/Intern or Practice).
Step 3 - Return the documents to us
As we don’t need the originals, you can scan the signed Appointment Letter and Insurance Information Form and email them to us at firstname.lastname@example.org. If you prefer, you can fax them back to us on (08) 6323 4668 or mail the originals to us at Suite 19, 23 Mill Point Road, South Perth WA 6151.
